It becomes the same problem as long-distance driving - after all, cars don't have built-in lavatories. For perspective: several times a year, I travel one of the busiest highways in North America. It has a number of food/fuel/rest stops along the section I travel. I only rarely see cars stopped along the shoulders for relief breaks, but the facilities at rest stops are well used. My point: as long as you don't try to go too far without a break, people can usually manage. $\endgroup$
